You might have to show a copy of your passport as well. You might also have to get an ISF bond. ($50), Customs Bond ($35), Customs Surety Bond (price timex 0.0065; minimum $100); there might also be a delivery order required to get the car released; you get this after all the agencies have had a look at it (not sure who actually issues it). You may also have to pay an escort fee into the port of entry (another hand is out for $50). The forms mentioned above are those that I used when bringing in a car from Sweden a little over a year ago.
